- Patients over 20 years old
- Form to request a copy of medical records (Download)
- Copy of patient ID card, driving license or passport
- Patients under 20 years old
- Form to request a copy of medical records (Download)
- Copy of patient ID card, driving license or passport
- Copy of ID card, driving license or passport of patient’s legal guardian or trustee
- Copy of the patient birth’s certificate (if no ID card, driving license or passport is available)
- Person authorized by patient to request medical records on their behalf
- Form to request a copy of medical records (Download)
- Copy of patient ID card, driving license or passport
- Copy of ID card, driving license or passport of patient’s legal guardian or trustee
- Signed letter of authorization from the patient and a copy of ID card, driving license or passport of the authorized person
